2. Dependents over the age of 18 enrolled in full-time school and living in the same household as the applicant, must include proof of Full Time Enrollment with the application (e.g. transcript, timetable or letter from education institution). A dependent over the age of 18 who is working and living in the same household as the applicant, must apply on their own and provide a copy of their own Notice of Assessment.

  • Canada Child Tax Benefit Notice
  • Goods & Services Tax/Harmonized Sales Tax Credit Notice
  • Valid Ontario Driver's License or Temporary Driver's License (only if accompanied by photo license card with same address)
  • Ontario Motor Vehicle permit (plate or vehicle portions)
  • Utility bill (home telephone, cable TV, public utilities commission, hydro, gas, water)
  • Valid Ontario Photo Card
  • Property tax bill
  • Insurance policy (home, tenant, auto, or life)
  • Mortgage, rental or lease agreement

NOTE: Newcomers/refugees who have landed within the last 12 months, MUST provide BOTH of the following documents:

  • Government of Canada Confirmation of Permanent Residence; OR Refugee Protection Claimant Document; and
  • Proof of Residency
