- 2025/2026 Reimbursement Form – Must be signed, dated, and submitted with all receipts. If receipts are not available, record each snow clearing service with the date, cost, and service provider's signature. This form is required for all applications, including online. Reimbursement cannot be processed without it.
- EFT Form – For residents who prefer direct deposit. Please complete and submit this form to:
